**Fighters: you must fill out a different form for each event you want
to fight in** YOU MUST FILL OUT A FIGHTER FORM IF YOU WANT TO
FIGHT IN OUR EVENT. In ADDITION to a fighter form, the State of
Michigan now requires ALL contestants to have a blood test within 12
months prior to fighting. ALL contestants must submit a copy of their
blood test results along with the fighter form. ALL contestants
MUST test NEGATIVE for: HIV, HEPATITIS B, and HEPATITIS C.
This paperwork can either be mailed, faxed or emailed. If we do
not receive both forms, YOU WILL NOT FIGHT!!!
